Registrations
If you wish to register with the practice, please ask the receptionist on duty for details. You will be given an application form and patient questionnaire to complete.
New patients will be asked to make an appointment with the practice nurse for a health check. When registering with the practice you can see the doctor of your choice if he/she is available, but please remember this may prolong the time you have to wait to be seen.
If you wish to join the surgery we will need to see proof of ID and some correspondence with your current address on. These documents need to be seen by us before we can register on the practice list.
